In order to safeguard students, the following identifying and background information is required of all volunteers who work with and around students on District campuses. 

Please submit the following..............

• Volunteer Form - Link
• TB Risk Assessment Questionnaire - Link - Effective 4 years based on the date the assessment was signed and stamped by a health care provider.
• TB Test Results - Effective 4 years based on the date the results were delivered.
• Valid photo ID - front and back.

All documents must be submitted to the office before starting to volunteer. Background/Security Information: BACKGROUND CHECKS ARE PERFORMED ON ALL VOLUNTEERS; THE PROCESS TAKES 24 HOURS. PLEASE CHECK WITH THE SCHOOL OFFICE TO CONFIRM CLEARANCE BEFORE VOLUNTEERING IN THE CLASSROOM. Anyone turning in their volunteer documents and wanting to volunteer the same day will be turned away.
